This is the first assembly that had 3 signal heads on the mast arm. Due to the width of the road, the arm is bolted to poles on both sides of the road. If it is a divided road, there may be a left turn signal either on the left pole, or on a separate pole all to itself if there is a divider between the left turn and through lanes.
