Twin double-red signals are used when it is desired to draw as much attention as possible to the red indication. It is rare to see double-reds on an overhead signal; the only occurrence I can think of is at the split-diamond interchange of 16 Avenue N (Trans-Canada Highway) and Deerfoot Trail (Alberta highway 2), which is given the dubious distinction of being the most dangerous intersection/interchange in Calgary. Signals like these are found on the off-ramps from Deerfoot Tr where they first intersect with 16 Ave (the vertical signal is on the near left corner); there is absolutely no excuse for going through a red light here, especially with the flashing yellow "prepare to stop" indicator near the beginning of the off-ramp. The following intersection has 3M PV signals (regular RYGs).
