Puppy Questionnaire

What this questionnaire is designed to do is give us an idea of what you are looking for in a Rottweiler, ie... personality, drive, show, working or pet quality, etc..  Please answer all of the questions as  honestly as possible and feel free to e-mail if there are any questions that I might clarify for you. The intent is not to pry into your life, but rather to get an idea of what kind of life we might be placing one of our puppies into.  This questionnaire is designed to help us pick the best possible puppy for your family.

Your Obligations

Are you willing to abide by a contract that includes things such as expectations, registration, non-breeding agreements, health and general care?

We REQUIRE that all of our puppies, pet or show quality, spayed/neutered or not, are FULLY health tested and certified through OFA and CERF (hips/elbows/eyes/heart) before the age of 27 months. This will require that our puppy's owners make arrangements and pay for these tests which may total close to $400+CDN.  Is this going to be something you are willing to do?  Do you have any questions regarding this health testing?

What are your plans for your puppy? (ie... showing, obedience trials, agility, breeding, house pet, etc...)

Please list below the names and phone numbers of two personal references of people that you have known for at least 2 years.

Do you have any other information you would like to tell us?  Do you have any comments?

Thank you for taking the time to fill out our puppy questionnaire.  It is important to us that we get to know our prospective puppy buyers before we even consider them.  This gives us a good idea of what a puppy's life might be like with you.  Please be aware that by filling out this form you have NOT YET been approved for the purchase of one of our puppies.  We need to have the time to go over it and when a decision has been made, we will contact you; usually within 72 hours of your submission.  Please also be aware that if the wait list exceeds the number of puppies in a litter that we will place puppies according to the date order of approved questionnaires.  You would be informed of this promptly however.
Thanks again for your interest in our dogs.  Please remember that quality puppies are worth waiting for!
