A message from the Author:
Over the years doctors have often
asked me if certain medical conditions were present in my family. Many times I would have
to answer: "I dont know." If I asked my mother she would reply:
"After seven children, I cant remember which one had what, or at what
age."
I decided that my four daughters should have
accurate information on their health as they grew up. So I started keeping records in
their individual "Baby" books. The problem was that these had limited capacity.
In very few years I was forced to rely solely on doctors records.
The convenience and extended hours of the Medi
Clinics were a godsend to a working mother. One problem I encountered was the way they
were set up. It wasnt always possible to see the same doctor on every visit. I felt
it unreasonable to expect the on-duty doctor to read the entire history of every client
for every appointment.
What was needed was a simple, convenient record
book that the patient could keep up to date, one that could cover a whole lifetime. Surely
such a record book existed, I thought. So I began my search and was surprised to find not
one. It was out of this experience that the idea for this book was born.
I started asking questions of friends,
family, doctors and ambulance attendants. Would such a record book be of significant value
and in what situations? What information would be the most helpful?
I was very encouraged by what I heard.
Doctors and emergency staff alike were excited about the prospect of such a personal
record. As time can be critical in many emergencies the accessibility of a personal
medical history could very well result in a life saved.
I designed and printed "My Personal Health Record Diary" for my family and myself and have recently
published it, making it available to you and yours.
