Home Support

We exceed what most traditional
home support programs offer. Our services may include:

▪    washing walls
▪    carpet cleaning
▪    removing clutter
▪    going to the dump
▪    cleaning all corners of a home

 

These duties go beyond the basic home care services of surface cleaning and bathing. We also maintain continuity for the client’s comfort.

▪    bathing/personal care
▪    medication reminders
▪    meal preparation
▪    house cleaning
▪    meal planning

 

When applicable and appropriate the client is encouraged to take responsibility for their home and we assist with housekeeping tasks rather than just do the work.

 

Community Support

     Many people with disabilities find it          difficult to get out and about to carry         out the day to day tasks of living, thus    furthering a sense of isolation. We will       assist and encourage routine activities       including but not limited to:

▪    banking
▪    shopping
▪    appointments
▪    correspondence
▪    budgeting
▪    bill paying

 

 

Recreation – Day Programs

Our team is creative and client focused. We have a well rounded knowledge of community supports and programs. The team is high energy, full of humor and ready to tackle anything. We are good listeners, good motivators and are professional and accountable.

We offer are one-on-one day programs tailored specifically to each clients
needs and desires.

Case Management

The case manager is the fixed and
central point of responsibility with
regard to information and service
coordination between the various
Ministries, professionals, family
members and friends who provide
service or support to the individual client
.
The case manager will provide an
Individual Program Plan followed by quarterly progress reports for review. We respect the individual’s rights, and maintain confidentiality and privacy.

The ACPD Assisted Living Program is
accepting new clients. We offer home support and rehabilitation, and case management. Having all facets of care working together allows for great communication, teamwork and
accomplishment of objectives. Care is based on an individual’s interests and choices. Maximize opportunities for community integration, and address social, cognitive, vocational, and emotional needs. We respect the dignity and individuality of each client

     

Costs
The total fee is based on individual client needs and the contract agreement. Our administration fee is 7% on the contract. Costs range from $27 - $34 per hour, based on needs of the client. Thank you for your consideration.
